Food Inspections: 8/22 - 9/4September 14, 2018
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.
These reports are from Aug. 22 to Sept. 4.
The Great Wall, 272 Brucke Strasse, Jasper: This establishment has corrected all 14 critical violations as well as 17 noncritical violations from the inspection on Aug. 18. They are expected to continue to maintain the food safety and sanitation standards set by the Dubois County Health Department and be on a unannounced inspection schedule of every three months.
Birdseye Dairy Barn, 8 State Road 145, Birdseye, one noncritical violation: observed several items in the sandwich prep table without labels telling what they are or the date they were prepared.
Breakaway Lanes and Recreation Center Inc., 104 E. 13th St., Huntingburg, no violations.
Chicken Place LLC, 4970 W. State Road 56, Jasper, three noncritical violations: food items (potatoes and chicken) in buckets stored directly on floor, growth of mold on beer cooler in bar area, accumulation of grease under fryer area in kitchen.
China Garden, 75 Industrial Park Road, Suite B, Ferdinand, two critical violations: raw foods stored above ready-to-eat vegetables in walk-in and reach-in cooler, observed raw food and coolor without covers in walk-in and reach-in coolers; one noncritical violation, observed improper thawing of fish.
China’s Best, 3669 N. Newton St., Jasper, (follow-up inspection conducted on Aug. 31 following Aug. 16 inspection), five critical violations: multiple foods measure in temperature danger zone, live bug found crawling on cold food buffet table, food observed being stored on floor in coolers, food observed uncovered in coolers, scooping utensils without handles observed laying directly in food items allowing for contamination of product, excessive dirt and food accumulation found throughout establishment; four noncritical violations, multiple broken tiles noted throughout the establishment, floor in walk-in cooler composed of a material that does not allow it to be easily cleanable, back door not providing protection from outer opening with multiple gaps and tight fitting, standing water accumulating in bottom of preparation coolor, cooling units throughout establishment did not contain temperature measuring devices.
Dairy Queen Grill & Chill, 1404 N. Main St., Huntingburg, two noncritical violations: observed several cold-holding refrigeration units with gaskets that are in very poor condition, also floor walk-in fridge appears to need repair, refrigeration units and topping tables appeared to have build-up food debris as well as non-contact food areas in the kitchen under grills and fryers and hot holding units presenting with food and grease build-up.
Los Dos Charros Mexican Restaurant, 406 E. 22nd St., Huntingburg, three critical violations: observed most nonfood contact services in the back with prolonged food and dirt debris throughout the dish room in the dry storage and in the walk-in fridge and on the shelves, noticed multiple dead insects in the dish room as well as several full traps in kitchen with active bugs in them, did not see any form of dating system throughout the facility.
Market Street Soup n’ Salad, 312 N. Main St., Huntingburg, no violations.
My Jalapeno Inc., 1107 N. Main St., Huntingburg, one noncritical violation: observed meat in reach-in freezer with no date or location from where it was purchased.
Ohana Hawaiian Grill, 3117 Newton St., Jasper, nine critical violations: observed employee not washing hands before placing gloves on hands, raw meats stored above and beside RTE foods in reach-in and walk-in coolers, observed live roaches in dry storage room, table top refrigerator contained food items out of temperature, items in all refrigeration units observed without any date marking, food items stored directly on floor and not covered in walk-in cooler, employee medicine stored in reach-in cooler next to and above food, walk, floors, microwave (kitchen and walk-ins) under equipment, shelving units, fans in walk-in coolers and freezers covered in dirt and food debris), refrigeration units (walk-in coolers, reach-ins) observed with dirt debris and water contamination; five noncritical violations, leaking ice machine and walk-in beer cooler causing water contamination, scoop in dressing at tabletop fridge was laying directly in produce and did not have a handle, holes in walls in dry storage area, mops observed not being hung to dry.
Orange Leaf, 3085 Newton St., Jasper, no violations.
The Rock Bar & Grill, 4997 W. State Road 56, Ireland, two noncritical violations, noticed slight build up of food and debris in the walk-in fridge, observed food in the facility not being consistently labeled name and date.
Yamato, 3015 N. Newton St., Jasper, five critical violations: live pests found in multiple areas of kitchen, foods observed without dates in walk-in and reach-in cooler units, food observed without covers in walk-in and reach-in coolers, establishment did not have required shell stock identification, refrigeration units observed to be soiled with dirt and food debris; eight nonviolations, nonfood contact surfaces heavily soiled with debris, extreme amount of mold accumulation found on walls near ice machine, mold accumulation observed in both ice makers, multiple broken and cracked tiles throughout establishment, cardboard observed being used as a contact surface, bulk items removed from original packaging did not have label with common name, food items observed being stored directly on the floor, knives were observed to be dirty that were intended to be clean, dish racks used to clean dishes were soiled debris, mop was not hung to dry.
Hopf’s Pies Plus, 3570 S. 1025E, Birdseye, no violations.
Huck’s #286, 230 Third Ave., Jasper, one noncritical violation: observed a couple of non food contact surfaces with food, dirt and debris (under coffee counter, Gatorade fridge and topping table, soda fridges), regular maintenance is recommended.
