Food Inspections: 6/24 - 7/3July 18, 2019
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.

Arby’s #5392, 3739 N. Newton St., Jasper, OK to operate.
China Wok, 317 E. Fourth St., Huntingburg, one critical violation: food observed being stored without covers. Four noncritical violations: multiple food items throughout establishment not labeled properly, cardboard observed being used as a contact surface on shelves throughout establishment, observed cans being reused for storage of food items, scoops being used did not have handles.
Headquarters Inc., 201 Main St., Jasper, no violations observed at time of inspection; inspector provided facility with a chart to guide in meat storage.
Kelley’s Restaurant & Bar, 6762 E. State Road 164, Celestine, no violations.
Little Caesars Pizza, 3121 Newton St., Jasper, one noncritical violation: observed gaskets on double glass door to be in poor condition.
Los Bravos Mexican Restaurant, 198 S. Newton St., Jasper, two critical violations: observed raw food not being stored separately from ready-to-eat foods in walk-in, food under prep tables did not have lids and were uncovered. Two noncritical violations: shrimp observed being thawed improperly, walk-in refrigeration unit leaking.
Mad Batter Bakery, 708 Main St., Jasper, no violations.
Main’s Bakery, 501 N. Washington St., Huntingburg, one critical violation: employee hand sink has no soap or paper towels.
Yamato, 3015 N. Newton St., one critical violation: food stored directly on floor in walk-in freezer. One noncritical violation: mold growth observed around ice machine and walk-in freezer.
Big Lots #254, 195 S. U.S. 231, Jasper, no violations.
Casey’s General Store #2385, 302 S. Main St., Huntingburg, no violations.
Circle S Mart #32, Huntingburg, no violations.
Dollar General Store #18995, 106 S. Main St., Huntingburg, no violations.
Ferdinand Processing Inc., 1182 E. Fifth St., Ferdinand, no violations.
Holiday Foods #3, 1815 Main St., Ferdinand, One critical violation: dust, dirt and mold buildup observed on shelves in juice area, on ceiling tiles in produce storage and fans/ceiling in bakery walk-in (significant food debris buildup observed in bakery). Two noncritical violations: observed failure to remove dead flies from bakery and juice areas, damaged walls/ceiling tiles observed in produce storage area.
• Comfort Inn, 123 Scenic Hills Court, Ferdinand, no violations.
• Holiday Inn Express, 2000 Hospitality Drive, Jasper, one noncritical violation: employee hand sink is being blocked by new cooking equipment, employees using three-compartment sink to wash hands. Establishment’s menu has changed, requiring them to have one employee with a Manager Serv Safe certificate.
• River House Bed & Breakfast, 410 Riverside Drive, Jasper, no violations.
• Sure Stay Plus, 951 Wernsing Road, Jasper, one critical violation: ready-to-eat breakfast foods, muffins observed to have no dating system.
