Food Inspections: 6/19 - 7/6July 31, 2018
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.
These reports are from June 19 through July 6.
American Table Family Restaurant, 1205 N. Main St., Huntingburg, 1 critical violation: observed multiple employee beverages being stored by ready-to-eat food and without lids to help from contamination; 4 noncritical violations, prepared lemons being stored in the ice holding bin that is used to serve customers, main ice machine used to produce ice presenting buildup of moisture and mold, noticed several areas missing ceiling tile along with heavy dust buildup on the ceiling vents and wires, observed multiple areas in the kitchen area storing different items without dates.
Bread & Butter Inc., 617 Main St., Jasper, 1 noncritical violation: employee personal food items in a location that can cause contamination to other items.
Eastown Recreation Center Inc., 1370 Crossroads Ave., Jasper, 4 noncritical violations: items in freezer and refrigerators not labeled, seals on pitcher/cup cooler in need of repair and cleaning, mold accumulation observed on ice machine, observed accumulation of dead pests in upright cooler.
Jasper Moose Lodge 1175, 2507 Newton St., Jasper, 2 noncritical violations: food found stored on floor in walk-in cooler, accumulation of dead pests found in back of establishment.
Little Caesars Pizza, 3121 Newton St., Jasper, 3 noncritical violations: noticed the double door glass walk-in fridge gaskets on both doors to be in poor condition, noticed hand washing sink had dishes sitting in it, observed mold buildup in the four-door pizza prep station, also noticed several dead insects in the soda cooler by the drive-thru window.
Los Bravos Mexican Restaurant, 198 S. Newton St., Jasper, 1 critical violation: multiple items found without lids (uncovered); 3 noncritical violations: refrigeration unit at end of cooking line had significant seal damage, mold accumulation found in small refrigeration unit and ice machine, mops found not being hung to dry.
Midwest Café & Market, 1102 Newton St., Jasper, 1 noncritical violation: observed no area for mop to air dry.
Miranda’s Restaurant, 1338 Newton St., Jasper, 1 critical violation: observed raw chicken being thawed on top of root beer bottles; 3 noncritical violations, bulk items being sold in smaller portions are being removed from original packaging without dates or where they are being manufactured, observed eggs in walk-in fridge being stored on the ground, noticed the can opener is in poor condition with severe rust.
Quality Inn, 406 E. 22nd St., Huntingburg, 2 noncritical violations: microwaves in customer area as well as kitchen/food prep area all soiled with food debris, nozzles of coffee dispenser soiled.
River House Bed and Breakfast, 410 Riverside Drive, no violations at this time.
Schnitzelbank Restaurant, 393 Third Ave., Jasper, 4 noncritical violations: items in walk-in cooler and freezer found not being stored at least 6 inches off the floor, observed items without a label in stand-up freezer, mold found on ceiling of walk-in cooler as well as ice machine, dust accumulation on ceiling and walls in cooking area specifically near vents.
Subway No. 10492, 75 Industrial Park Suite A, Ferdinand, 3 noncritical violations: cutting boards showing heavy scoring, single service items found being stored directly on floor, food and nonfood contact surfaces throughout establishment showing accumulation of food particles.
Subway No. 2776-Sharma Inc., 2506 Newton St., Jasper, 1 critical violation: observed food employee handle customer transaction and then place gloves on hands to handle food and did not wash hands in between tasks; 4 noncritical violations, drain for walk-freezer very clogged with mold/food debris allowing for standing water; cutting boards in back and on right side of front line have very deep scoring and becoming back in the grooves, mold accumulation on soda dispenser in lobby, hand towel dispense empty and not conveniently located hear hand wash sink.
Taqueria El Illano, 111 W. 28th St., Apartment B, Jasper, 2 noncritical violations: observed light in walk-in fridge with no shield covering the bulb, noticed multiple items being stored without dates or food labels.
Wendy’s No. 274, 1209 N. Main St., Huntingburg, 1 critical violation: hand wash sink observed to have food particles in it; two noncritical violations, pest accumulation in establishment, most notably near drive-thru area, establishment had large accumulation and grease and food debris throughout entire kitchen area.
The Parklands (pool), 800 W. 15th St., Jasper, 2 violations, residual disinfectant levels above maximum allowed, alkalinity outside acceptable range of 80-120 ppm.
Casey’s General Store No. 3504, 2245 Main St., Ferdinand, no violations.
Chuckles/Shell Food Mart No. 20, 620 W. Sixth St., Jasper, 1 critical violation: chemical sanitizer in sanitize sink observed not to be registering on sanitizer strip; 3 noncritical violations, food particle accumulation observed on roof of microwave oven, multiple cutting boards on front line showed deep scoring, observed accumulation of food particles/dirt throughout establishment.
Circle S Mart No. 33, 850 Main St., Ferdinand, 4 noncritical violations: items found stored on floor in back room storage, mop found not being hung to dry, vents in walk in cooler showed excessive accumulation of dust, nozzles of cappuccino and coffee creamer dispensers had large amounts of accumulation.
Dollar Tree No. 01349, 3601 Newton St., Unit 101, Jasper, 2 noncritical violations: food storage items were seen on the floor, observed one of two mops not being air dried.
Jasper Community Arts, 951 College Ave., Jasper, no violations.
Kalb’s Crossroads Service, 5130 E. State Road 56, Dubois, no violations.
Ruler No. 261, 155 E. 30th St., Jasper, no violations at this time.
Sternberg 24-N-More, U.S. 231, Jasper, 1 critical violation: raw hamburger patties stored above drinks in walk-in fridge; 2 noncritical violations: ice machine, ceiling vents and blue ice cream freezer appear to have mold buildup, also vents and fans in cooking area have dust buildup, general cleaning of cooking area under and behind grill and fryers.
Comfort Inn, 123 Scenic Hills Court, Ferdinand, 1 noncritical violation: observed a significant buildup of debris on nozzles of juice machine.
Indulge, 1800 S. 800W, Velpen, no violations.
