Food Inspections: 5/16 - 5/29June 12, 2018
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.

Columbian Home Association (Knights of Columbus), 201 E. 30th St., 1 noncritical violation: light bulb shield missing from walk-on freezer.
Deb’s Truck Stop, 502 W. State Road 64, Birdseye, 6 critical violations: significant grease and food buildup on non-food contact surfaces throughout establishment, dead pests found in kitchen area, observed single use food items being stored directly on floor, raw food items being stored above ready-to-eat foot items in all refrigeration units, observed bare hand contact with food items, food items stored in refrigeration units did not have a cover, observed improper hand washing procedures; eight noncritical violations, refrigeration unit handles, drawer handles in inside refrigeration units need to be cleaned, observed improper hand washing procedures, sandwich prep table and dish washing unit not functioning as intended, can opener soiled with food debris and metal shards (needs sharpened), refrigeration units not all equipped with thermometers, integrity of counter tops, ceilings and floors compromised through chipping, mop was not being hung to dry, establishment does not have a service sink, mop water being discarded outside.
Denny’s #377, 3850 Newton St., Jasper, 6 critical violations: four of five hand wash sinks contained items indicating being used for things other than hand washing, pull out fridge in middle of line found to have cooked and raw meats, observed employee applying gloves without previously washing hands, observed clean plates on serving line containing food particles, front line food prep equipment contained moderate amounts of food debris; 6 noncritical violations, shelving units on serving line contained food debris and particles, walls, hood and microwave contained debris, air vents, floors, walls and ceilings contain food particles and debris buildup, clutter observed in dry storage near the soda machine, three towels used to wipe food and nonfood contact surfaces observed not being stored in sanitizer solution, cooks on front line were without food thermometers, two boxes of packaged foods found on floor in freezer unit, one wet mop found lying directly on floor, second mop was in contact with bucket.
Huntingburg Country Club Inc., 739 W. Third St., Huntingburg, 1 noncritical violation: meat in freezer not labeled properly.
McDonald’s #575810, 25 Industrial Park Road, Ferdinand, two critical violations: food items found in hand wash sink near back drive through area, sanitizer bucket did not contain actual sanitizer because establishment ran out.
Jasper Dog Haus Inc., 155 Second St., Jasper, okay to operate.
Long John Silvers #70196, 3960 E. Newton St., Jasper, five critical violations: vents through establishment showed dirt accumulation, behind fryers showed significant grease accumulation, underneath of front serving counters showed large amount of food debris, tiles in front of fryer showed severe damage and cracking, refrigerator/freezer near fryer had very rusted/dirty holding racks; one noncritical violation, observed mold buildup on front and back ice maker.

Red Roof Inn & Suites, 440 S. Main St., Ferdinand, 1 noncritical violation: no temperature measuring device in small refrigerator near microwave.
Sisters of Saint Benedict, 802 E. 10th St., Ferdinand, no violations.
Taco Bell #3001094, 420 S. Main St., Ferdinand, 2 noncritical violations: drive-thru soda station had heavy soda buildup, food preparation line had food debris buildup.
Wendy’s, 480 S. Main St., Ferdinand, two critical violations: hamburger refrigeration unit observed to be between 47-51 degrees, hand wash sink not reaching 100 degrees; six noncritical violations, two mops observed to be drying on floor, tomatoes and other items in walk-in cooler not labeled with date, employee observed not wearing hair restraint in food preparation area, food establishment showed general overall buildup of food debris and grease, seals on multiple refrigeration units appeared to be ripped, soft serve in walk-in cooler possibly being exposed to contamination via raw hamburger patties, lettuce in walk-in cooler had potential of being contamined by soft serve due to how it was being stored.
Cash Saver #1174, 3605 N. Newton St., Jasper, 1 critical violation: several dented cans found throughout the store; noncritical violations; 5 noncritical violations, pastry items stored on floor by produce and multiple items being stored in walk-in freezer, four-door ice cream freezer producing heavy ice buildup, employee food being stored in walk-in produce (needs identification if employee’s or store’s), both employee bathrooms hand washing sinks have hot water temp of 94, women’s room trash can is missing a lid.

CVS Pharmacy #6871, 20 Industrial Park Drive, Ferdinand, 1 critical violation: significant mold buildup on ceiling, racks, and so forth, in all freezer-refrigerator units in facility.
CVS Pharmacy #6878, 617 W. Sixth St., Jasper, 1 noncritical violation: thermometers missing from three double-door fridges.

Dollar General Store #2144, 1710 N. Main St., Huntingburg, 1 critical violation: observed multiple severely dented cans in the canned good area; 1 noncritical violation, mop was observed not being hung to dry.
Dollar General Store #4509, 4117 N. Mannheim Road, Jasper, 3 noncritical violations: unisex bathroom is missing a lid, both mops need to be able to hang dry, single door fridge is missing thermometer.
Dollar General Store #7817, 131 E. 10th St., Ferdinand, two critical violations: multiple severely damaged cans found in canned food aisles, excessive live bugs found in ice cream freezer at front of store; 1 noncritical, dead bugs found in drink cooler near check out and ice cream freezer near entrance.

Family Dollar #29125, 705 N. Main St., Huntingburg, 1 critical violation: multiple cans found to be dented in canned goods aisle; 3 noncritical violations, mop found not being hung to dry, underneath of aisle shelving contains large amounts of debris, dirt and food items, bathroom used by females did not have a covered receptacle.

Hometown IGA 460, 312 E. 12th St., Huntingburg, 4 critical violations: items in salad bar not being held at proper temperature, multiple cans found to be dented in the canned food area, raw meat in food cooler not covered (food items found being stored on floor of walk-in cooler), contaminated knife being stored on boxes with food in meat cooler; two noncritical violations, food contact and noncontact surfaces soiled with debris and food particles in deli, walk-in coolors, and so forth; hand wash sinks in deli and meat area did not have paper towels for the hand wash sink.
