Food Inspections: 4/3 - 4/18April 29, 2019
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.

Applebee’s, 4223 Mannheim Road, Jasper, no violations.
Hardee’s Restaurant, 726 Second St., Jasper, one noncritical violation: observed some areas in kitchen with built-up food and debris, in fridge storage units, behind equipment and lower shelves.
Hot Spot Bar & Grill LLC, 5416 E. Main St., Dubois, two critical violations: multiple food items observed without proper date marking in reach-in refrigerators, food items observed uncovered in reach-in refrigeration units.
Kountry Lane Bakery, Second and Newton streets, Jasper, no violations.
Los Dos Charros Mexican Restaurant, 406 E. 22nd St., Huntingburg, one critical violation: observed some product without a date, also no way of tracking where it came from and when; one noncritical violation: walk-in fridge condenser fans have dirt and debris buildup.
Ohana Hawaiian Grill, 3117 Newton St., Jasper, one critical violation: observed some raw meats, hamburgers, fish stored above ready-to-eat products in two-door refrigerator; one noncritical violation: observed the grill hood system to have heavy buildup.
Taco Bell #3001094, 420 S. Main St., Ferdnand, no violations.
Villa Pizzeria, 124 Third Ave., Jasper, two critical violations: observed all refrigeration units to have food, dirt and debris buildup, kitchen area and equipment presenting heavy food and dirt accumulation, floors, hoods, walls, shelves, prep tables; four non-critical violations: equipment that is not being used in the back storage area should be removed, industrial can opener has built-up food and dirt and also blade might need to be replaced, product that has been removed from original packaging has no labeling to identify where it came from and when, walk-in refrigeration unit’s condenser is leaking and causing possible contaminations. All critical violations have been corrected.
Wendy’s #340, 480 S. Main St., Ferdinand, no violations.
Wings Etc., 3683 N. Newton St., Jasper, three critical violations: hand-washing sinks (two) in kitchen area observed being used for things other than hand-washing, sanitizer concentration throughout establishment observed to be extremely low, foods observed to be uncovered in reach-in refrigerator near fryers; one noncritical violation: hand-washing sink near ware-washing machine only reaching 90 degrees F.

Chuckles/Shell Food Mart #21, 5498 E. Main St., Dubois, one noncritical violation: debris buildup observed on soda machine nozzles.

Huck’s #339, 601 N. Main St., Huntingburg, four critical violations: several ready-to-eat deli items observed out for the public to purchase with expired use-by dates, as well as milk and eggs; prep and cooking areas throughout the whole kitchen seen to have built-up food, dirt and debris; all refrigeration and freezer units throughout the entire kitchen area, as well as all walk-in fridge units have very heavy signs of food, dirt and debris buildup on the doors, shelves and floors; several items that are for food production and storage being stored directly on the floor. Two noncritical violations: observed multiple items in the back storage walk way by bag in the box station cluttering up the area; soda station observed to have some buildup where ice is received and around nozzles.
Jasper Marathon (D-Rock Inc.), 1908 E. Newton St., Jasper, two noncritical violations, observed buildup of dust and debris in fan/seals in Sunkist refrigerator, observed buildup of debris on dispensing nozzles of drink machines.

Quality Inn & Suites, 440 S. Main St., Ferdinand, one noncritical violation: dispensing nozzles of juice machine showed buildup of juice.
