Food Inspections: 2/26 - 3/14March 22, 2019
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.
These reports are from Feb. 26 through March 14.
Arby’s #5392, 3739 N. Newton St., Jasper, one critical violation: observed the kitchen-area floors to be heavily soiled with food, dirt and debris, as well as the walk-in refrigerator and freezer, also the storage fridges below the sandwich building station are presenting signs of food and debris buildup.
Baker’s Corner, 11018 E. State Road 56, French Lick, no violations.
Breakaway Lanes and Recreation Center Inc., 104 E. 13th St., Huntingburg, no violations.
China’s Best, 3669 N. Newton St., Jasper, one critical violation: sushi temperature observed at 58 degrees F.; one noncritical violation, heavy dust accumulation on fan in walk-in cooler.
El Tapatio Mexican Restaurant, 101 Place Road, Jasper, one noncritical violation: product that is moved from its original container into something else should be labeled to properly identify what it is and where it came from.
Fazoli’s #1675, 703 Third Ave., Jasper, two critical violations: observed dirt, debris and food buildup in all of the refrigeration units in the kitchen area, topping tables, upright fridge and freezer; observed heavy concentration of dirt, debris and food buildup throughout the back of the facility, prep tables, clean-dish holding station and shelving units. Four noncritical violations: air vents, floors, walls and ceilings showed signs of food, dirt and dust buildup; some boxes with food-storage containers seen being stored on the floor (such items should be stored 6 feet off the floor); tiles in walk-in fridge falling apart, as well as some ceiling panels noticed to be missing; did not see a mop drying process to prevent it from properly drying.
Happy Hour Bakery, 914 S. Celestine Road S., Schnellville, one critical violation: PHF observed at approximately 52 degrees F. in walk-in cooler. Two noncricital violations: walk-in cooler observed not being in good repair (not cooling adequately); freezer/cooler observed not equipped with temperature measuring device; mold growth observed in ice machine.
Homestead Pizza LLC, 1510 Main St., Ferdinand, no violations.
Jasper Dog Haus Inc., 155 Second St., Jasper, no violations.
My Jalapeno Inc., 1107 N. Main St., Huntingburg, one critical violation: observed several items in walk-in fridges without dates of preparation or expiration. One noncritical violation: a couple of ceiling panels were observed to be falling apart.
