These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Brew Jasper LLC, 408 Main St., Jasper, 1 noncritical violation: small fridge under coffee bar with cold coffee, chocolate syrup and metal steamer pots has temperature at 50 degrees (no potentially hazardous foods are currently being stored in that location).
Brick Oven Pizza/Chester Chicken, 1281 Third Ave., Jasper, 1 noncritical violation: observed potato slicer with food buildup and can opener.
Grandy’s, 3891 N. Newton St., Jasper, 2 noncritical violations: nonfood contact surfaces observed to have built up dirt, grease and food debris (casters, bottom shelves, carts, DT soda station, tray holding), noticed a few tiles missing or broken in front of fryer station.

J.R.’s Bar (Taylor Michaels Inc.), 23 S. Clay St., Jasper, 1 critical violation: sandwich prep table has a temp of 50, management will monitor it; two noncritical violations, no dedicated hand washing sink in the facility, observed meat product being thawed at room temperature.

Long John Silver’s #70196, 3960 N. Newton St., Jasper, 2 noncritical violations: observed food and grease buildup behind fryers underneath front counters, drive-thru soda station and four-door fridge/freezer noticed having buildup.
McDonald’s #575810, 25 Industrial Park Road, Ferdinand, 1 critical violation: food debris found in hand wash sink near dish washing station; 2 noncritical violations, hand wash sink on southwest wall did not have hot water (maintenance was called while onsite), food debris build up on nonfood contact surfaces throughout establishment.
Red Roof Inn & Suites, 440 S. Main St., Ferdinand, 1 critical violation: single service food contact items observed being stored directly on the floor.
Taco Bell #3001094 (Ferdinand), 420 S. Main St., Ferdinand, 1 noncritical violation: food debris found on nonfood contact surfaces.
Wendy’s #340, 480 S. Main St., Ferdinand, 1 critical violation: cut tomatoes observed at approximately 50 degrees.
Wendy’s #384, 3565 Newton St., Jasper, 1 critical violation: observed cut lettuce at 51 degrees, chili measured at 123 degrees; four noncritical violations, single-service items (cups) stored directly on floor, broken tiles observed in front of fryers, mop found not being hung to dry (service sink needs to be cleaned), observed mold growth in ice machine and under refrigeration units in sandwich building line.

Note: Make sure all food refrigeration units are equipped with a temperature-measuring device (one not seen in the deli meat and cheese refrigerator).
Dollar General Store #7817, 131 E. 10th St., Ferdinand, 2 critical violations: spoiled milk buildup found in reach-in cooler, drink racks accumulating dirt and dust in reach-in coolers and soda coolers, severely damaged cans found on multiple shelves throughout establishment.
