Food Inspections: 1/10 - 1/23February 12, 2018
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.
These reports are from Jan. 10 through Jan. 23.
Azura Restaurant Systems Inc., 198 Third Ave., Jasper, no violations observed.
Brick Oven Pizza, 420 E. Sixth St., Huntingburg, one critical violation and one noncritical violation. Critical: Spray bottle containing sanitize chemical did not have proper concentration; corrected by Jan. 16. Noncritical: North door entering into the kitchen was not sealed to the exterior; corrected by July 16.
China Garden, 75 Industrial Park Road, Suite B, Ferdinand, two critical and one noncritical violations. Criticals: Raw fish stored on top of ready to eat lo-mein noodles and an employee was seen eating at large stainless steel prep table in kitchen area. Both violations were corrected. Noncritical: Utensils used for scooping product must have a handle and the handle must be stored in a manner that does not allow hands to contaminate the product. Violation needs to be corrected.
Libby’s Gourmet Ice Cream, 605 Main St., Jasper, one critical and two noncritical violations. Critical: Inside of cheese container shows visible soil; corrected by Jan. 20. Noncritical: Top of milk shake blender shows visible soil; corrected by Jan. 20; light bulb burned out in cheese/meat refrigerator; corrected by Feb. 2.
Mad Batter, 708 Main St., Jasper, two noncritical observations. Employees observed without effective hair restraint; front display fridge had a small amount of ice accumulation on coils. Corrected by Jan. 24.
Mi Jalisco, 239 Baden Strasse, Jasper, two critical and two noncritical violations. Criticals: Sanitizing chemical in dish machine did not measure to appropriate strength and the sanitizing chemical bucket needed to be replaced because it was out of product; raw chicken was found to be stored over ground beef in walk-in refrigeration unit and above ready-to-eat vegetables in refrigeration unit. Both violations were corrected. Noncriticals: Missing ceiling tiles in dish area and above hot water heater; employee restroom hand sink had no hot water because valve was leaking excessively. Both violations were corrected.
My Jalapeno Inc., 1107 N. Main St., Huntingburg, two noncritical violations. Microwave is soiled with dried food debris and grease and dish machine’s sanitizing chemical dispenser was not dispensing. Corrected by Jan. 16.
Subway-Bounds Enterprises Inc., Huntingburg, no violations observed.
Susan’s Catering/Pizza Place, 204 Ohio St., Holland, no violations observed.
The Great Wall, 272 Brucke Strasse, Jasper, one critical and 10 noncritical violations. Critical: Walk-in cooler unit and long reach-in cooler had recorded temperatures of 56 degrees and 53 degrees, respectively; problems corrected from inspection on Jan. 17. Noncritical: Bowls being used to scoop flour and hood ventiliation filters being full of grease, problems corrected. Hot water had incorrect temperature; corrected by Jan. 24; cooking staff not wearing hair restraints, corrected by Jan. 19; non-food contact surfaces are dirty and have grease on them; corrected by July 17; floor tile cracked and missing; corrected by Aug. 17.
Tres Agaves Mexican Grill, 1935 Main St., Ferdinand, two noncritical violations. Improper thawing procedures for fish filet and steaks; utensils used for scooping product must have a handle and stored in a manner to now allow hands to contaminate the product. Both violations were corrected.
Big Lots No. 254, 195 S. U.S. 231, Jasper, no violations observed.
Casey’s General Store No. 2385, 302 S. Main St. Huntingburg, two noncritical violations. Microwave was soiled on the ceiling of the cavity and disposable towels were unavailable at hand sink. Both were corrected.
Casey’s General Store No. 3504, 2245 Main St., Ferdinand, no violations observed.
