Food Inspections: 10/27 - 11/19November 26, 2018
These summaries of the Dubois County Health Department’s inspection reports are made public after the restaurants and food stores have had 10 days to respond. Reinspections are at the inspector’s discretion. Violations are corrected on the day of the inspection unless otherwise noted. If you have questions, contact the health department at 812-481-7055.

Applebee’s, 4223 Mannheim Road, Jasper, no violations.
Hardee’s Restaurant, 726 Second St., Jasper, three noncritical violations: observed built up of food debris in the microwave ovens and in the back kitchen area, noticed mold build up in the ice storage compartment, walk-in fridge missing shield on light fixture.
Holland American Legion (Harmeyer Post 343), 508 W. Main St., Holland, two noncritical violations: food observed to be thawing at room temperature, blade on can opener is worn and metal is chipping. Observed topping prep table at 45 degrees, manager is monitoring temperature to assure it hold at 41 degrees or below.
Hot Spot Bar & Grill LLC, 5416 E. Main St., Dubois, four noncritical violations: nonfood contact surfaces (floor, counters, handles of sink and fridge, walls behind cooking area) observed to have accumulation of food particles, uncovered potatoes in two-door reach-in refrigerator, food observed to be unlabeled in two-door reach-in and pizza topper, growth of mold observed in beer cooler in bar area.
Ohana Hawaiian Grill, 3117 Newton St., Jasper, all violations from the Sept. 6 visit were observed to be corrected.
Old School Café, 304 E. Fourth St., Huntingburg, one noncritical violation: light bulbs above stove top appear to be not shatter proof.
Soup-N-Such Bistro, 1150 Main St., Ferdinand, no violations at time of inspection.
Wings, Etc., 3683 N. Newton St., Jasper, one noncritical violation: foods found to be uncovered in reach-in freezer near fryers.
Chuckles/Shell Food Mart #21, 5498 E. Main St., Dubois, one noncritical violation: mold growth observed in reach-in drink refrigeration unit in northwest corner.
CVS Pharmacy #6881, 610 N. Main St., Huntingburg, no violations at this time.
Dollar General Store #2144, 1710 N. Main St., Huntingburg, no violations at this time.
Family Dollar #31267, 2 E. State Road 64, Birdseye, one critical violation: several dented cans found on various shelves.
Fast Track 500, 1307 N. Main St., Huntingburg, okay to operate.
Grounded, 435 S. U.S. 231, Jasper, pre-operational inspection: location change, okay to operate.
Jasper Marathon (D-Rock Inc.), 1908 N. Newton St., Jasper, two noncritical violations: water from sinks in establishment noted not reaching at least 100 degrees F. (80-95 degrees F.), cavity of microwave observed to have a significant buildup of food debris.
Jasper Skate Palace Inc., 1013 Fourth Ave., Jasper, two noncritical violations: establishment not equipped with a service sink; can opener observed to be rusted, chipped and dirty.
Kountry Lane Bakery, 2nd and Newton, Jasper, no violations. All packaged food items need to have an ingredient label either on them or on an ingredient card near the product.
One Stop Convenience, 500 W. State Road 64, Birdseye, no violations at this time.
